Release of Liability and Waiver
By signing this form, I acknowledge and agree to the following:
1. Acknowledgment of Risks: I understand that participation in pickleball involves physical activity and carries inherent risks, including the risk of injury.
2. Release of Liability: I release Crew Pickleball Club, its staff, instructors, and facilities from all liability for injury, damage, or loss arising out of my child’s participation in the program.
3. Medical Authorization: In the event of an emergency, I authorize the program staff to secure necessary medical treatment for my child and agree to assume all financial responsibility for such treatment.
4. Behavior Expectations: I agree to ensure my child adheres to the program’s behavior guidelines. I understand that failure to do so may result in their removal from the program without a refund.
5. Photography Consent: I understand that signing and submitting this registration form means that I consent to my child’s photo/video being used for program promotional purposes.
6. Registration Fees: Registration fees are non-refundable. If a player withdraws, Crew Pickleball Academy will provide account credit for future programs when possible.
